Coding femoral acetabular impingement syndrome

Presently there is no direct diagnosis code for FAI lesion.  Here is an example of coding FAI lesions in CPT. Before looking into the below example you should understand the CPT codes for FAI lesion and here is a link for it FAI lesion coding

EXAMPLE:

PREOPERATIVE DIAGNOSES:
I. Right hip femoral acetabular impingement syndrome.
2. Right hip labral tear.

POSTOPERATIVE DIAGNOSES;
1. Right hip femoral acetabular impingement syndrome.

2. Right hip labral tear plus synovitis.

PROCEDURES PERFORMED; Right hip arthroscopy with:

1. Acetabular rim trim.
2. Proximal femoral osteoplasiy.

3. Labral debridement.

4. Labral repair.

5. Synovectomy.

Dx      Code      Description
1        719.85    FAI Hip
2        718.05    Hip labral tear
3        727.09    Hip Synovitis/Tenosynovitis

CPT   Code   Description                                    Modifier
1        29914  Hip Arthroscopic  Femoroplasty     RT
2        29915  Hip Arthroscopic Acetabuloplasty   RT
3        29916  Hip Arthroscopy Labral Repair       59-RT

Although CPT code 29915 includes labral repair, but the physician is doing labral repair due to labral tear and it is not incidental so we can code CPT 29916 along with 29915 with 59 modifier.
